The Office of Early Childhood has received stimulus relief money from the Federal Government to help support early childhood programs in Connecticut who have faced challenges due to the COVID-19 pandemic. We plan to distribute this funding to child care programs like yours, and the application process will be announced soon. To allow us to make payments to your program, we will need an IRS W-9 form from you. The State Comptroller will use the information on the W-9 to issue a Supplier ID so any payments that you may be eligible for can be paid to your program.
To expedite the issuance of a Supplier ID, download the IRS W-9, and then upload your completed W-9 along with the other requested information below. You will receive email notifications once we have begun processing and when we have verified your information.
